Wellness IV Program
Our Wellness IV formulas are not intended to treat any medical condition, but rather offer a boost to your overall vitality and sense of well-being within a targeted goal. Patients can take the lead on choosing their preferred formula, while the doctor provides guidance, education and a safe environment throughout your IV visit. For individuals without an underlying medical condition, you may elect to schedule a Wellness IV with one of our providers without a prior office visit and blood work. Individuals with an underlying medical condition must first schedule an office visit and complete blood work (within 1 year), prior to receiving any type of IV therapy at FoRM Health PDX in Portland, Oregon.

Our Medical IVs are intended for those seeking improvement in symptoms related to a specific diagnosis or condition. Prior to receiving any Medical IV, an office visit and current blood work (within 1 year) is required.

FAQ
IV ozone therapy, otherwise known as Major Autohemotherapy (MAH), involves the administration of medical grade ozone into the body through a sterile, safe and efficient process. It’s often used to improve fatigue, brain fog and reduce symptom flares associated with autoimmune conditions and chronic infections. It’s a safe + natural therapy to help improve immune function, support use of oxygen and red blood cell function, increase antioxidant levels and decrease inflammation.
High-dose vitamin C therapy is the administration of vitamin C at a higher concentration than our standard IV Immune Boost. It combines (up to 25 grams) of Vitamin C with other supportive vitamins and minerals to stimulate & support the immune system in the face of chronic illness. This option is considered for clients struggling with recurrent respiratory infections, chronic fatigue from persistent infections (Reactivated Mononucleosis or Long Covid), or as a supportive therapy in mold protocols.
Iron infusions are indicated in persistent iron-deficient anemia when oral therapies are either insufficient, not tolerated well, or not indicated. It can provide a quick resolution of symptoms secondary to chronic iron deficiency, including fatigue, weakness, dizziness and brain fog. There are two options for iron replacement solutions, which will be discussed at your initial office visit prior to receiving IV iron.
NAD+ is a coenzyme that is found in all our cells that supports energy levels (mitochondria function) and promotes longevity (DNA + cellular support). As we age, NAD+ naturally decreases, which negatively impacts energy levels, brain function, metabolism and overall vitality. NAD+ IV therapy is the most efficient way to support cellular repair and function, reduce oxidative stress, and promote longevity.
Custom IVs are for patients who would benefit from a more tailored approach, such as those with chronic, complex conditions. IV solutions are formulated based on labs such as Micronutrient deficiency +/or Organic Acids testing. These tests can help identify hidden nutritional deficiencies which may be contributing to symptoms. These IVs may also be considered if you are interested in a specific infusion that is not listed as an option under either Wellness Boost or Medical IVs.
IV nutritional therapy involves administering vitamins, minerals, antioxidants, electrolytes, amino acids + other health-promoting compounds directly into your bloodstream using a small catheter inserted into a vein.
Our IV Boost formulas are safe for anyone without an underlying medical condition. IV therapy may not be appropriate for individuals with a significant fear of needles. If you are someone with a chronic medical condition, you will need to establish care with a FoRM naturopathic doctor to discuss your health history and current needs to determine whether you are a good fit for this treatment.
There are a few patients who might not be a good candidate for IV therapy in general, including patients suffering from cancer, liver +/or kidney disease, heart failure, or blood clotting disorders.
IV nutritional therapy allows us to deliver nutrients more efficiently, effectively, and at higher concentrations than what standard oral supplementation typically allows. Since it is delivered directly into the blood stream, you are able to absorb 100% of the nutrients provided.
